This program provides wraparound services to adults 19 and older that are Nebraska residents that may be struggling with severe persistent mental illness or substance abuse issues. This free program also provides socialization opportunities, help with coping skills, health and wellness information, daily living skills such as cooking and cleaning, medication management, case management, transportation to and from medical appointments. Services also include resources for housing, food stamps, nutritional, and medication assistance as well as other social services in the community. Heartland strives to reduce the need for higher level of care for these individuals, such as hospitalization or legal involvement, saving the counties, tax payers, and families additional expenses that they do not have the resources to pay for.
